New Client Paperwork for Therapy

We use a secure online platform specifically created for therapy practices called TheraNest. We appreciate if you can complete the necessary paperwork digitally ahead of time before your first scheduled session so that we may review it before we meet as well as save on paper and printing. Your therapist will send you an emailed link to join the Client Portal before your first session where you can access, sign, and submit the paperwork.
